Main duties of the job

1.Patient identification using the Eclipse risk stratification tool.

2.Comprehensive review of primary and secondary care records (including recent investigations, UECC/hospital attendances, and admissions).

3.Telephone-based reviews with the patient and/or carer - particularly in severe frailty or where the patient lacks capacity to participate.

4.Focus on community-dwelling older people (including care home residents).

5.Correspondence to primary/secondary care and social care (as appropriate) to align and/or rationalise ongoing medical review and support.

6.Advance Care Planning, including completion of ReSPECT documentation.

7.Polypharmacy review and medicines optimisation with support from medicines management.

8.Liaison with Falls, Continence, Hospital at Home, Community Matrons, and Primary/Secondary Care clinicians.

9.Data collection for quality improvement and service evaluation.
