Royal Primary Care Brooklyn is seeking an experienced healthcare assistant with an interest in frailty to support our multi-disciplinary team of professionals – including working alongside our colleagues in the ARCH PCN (Alfreton, Ripley, Crich and Heanor) and other stakeholders in the community.

This is a developing role within our practice team providing a new service to all housebound, vulnerable and frail patients. This role will also involve working alongside GPs and Advanced Clinical Practitioners aligned to care homes, supporting with HCA tasks, the weekly ward round and annual long-term condition reviews as required.

We want to improve the health of our practice population and provide better access to healthcare for those who may struggle to visit the practice, reducing health inequalities in our area.

Main duties of the job

You will work collaboratively as part of our multidisciplinary (Heanor) team of clinicians to meet the needs of our patients; delivering the highest standards of patient care.  You will undertake a diverse range of assessments and provide the appropriate care / treatment according to patient needs, striving to improve standards of care and enhancing nursing services within the practice.

Depending on experience, contribution to the monitoring of Long-Term Chronic Disease management of our patients will also form a large part of the role. Knowledge of the GMS contract e.g. The Quality and Outcomes Framework will be valuable to the post.
