Job overview

You will be part of a well established dynamic team that delivers a programme of stroke rehabilitation to patients in their own homes.

The team is designed to provide therapy at the same level of intensity at home as in hospital.

The team comprises of Physios, OTs, SLTs, Nurses and other rehab workers.

You will be supervised by a team of therapists who will devise rehab plans which you will follow as a lone worker.

Main duties of the job

You will deliver therapy plans around mobility, personal care and communication and swallowing. They will be orientated around the patients own goals that will be geared to increase independence as they progress.

The therapy is delivered in patients own homes immediately after discharge, the service is 7 days per week 365 days per year.

You will be a confident lone worker with some opportunities for joint sessions when appropriate.

Some knowledge of working with people who have  experience stroke is desirable.

Detailed job description and main responsibilities

To facilitate rehabilitation through delivering nursing and therapeutic care allowing patients, post stroke, to gain independence within their home environment. To promote the best possible care, within the patients care plan, to aid health, hygiene, comfort and dignity for all patients while implementing a stroke rehabilitation package.

  • Work unaccompanied, in the patients’ own home, to deliver a rehab programme through nursing and therapy tasks to patients following a stroke.
  • Carry out the rehab plan as set out by all nursing and allied health professional staff involved in the care of an individual patient.
  • Directly responsible to the Early Supported Discharge Team Co-ordinator.
