We are looking for people that are kind, compassionate, creative, problem solvers, with excellent communication skills, to join our expanding team across the York and South Hambleton and Ryedale areas. Within your role you will form a crucial part of the Multi-Disciplinary Team, delivering a high standard of health care to patients living within their own homes, facilitating short term rehabilitation, reablement and recovery in order to avoid unnecessary hospital admissions and enable patients to recover following an acute hospital stay.

You will work closely with patients and their families, nursing and therapy colleagues within the team, and will build connections with health and social care partners across the local system. Taking a re-ablement approach the service supports people to return to, and / or, remain in their own home.

We are looking for candidates with the ability to motivate individuals, to aid their recovery and maximise independence in order to achieve the goals that matter to them. Roles are available across York and South Hambleton & Ryedale areas.

Main duties of the job

You will undertake duties delegated by the Nursing, Physiotherapy and Occupational Therapy teams, and will be responsible for providing holistic, person centred, compassionate health care to patient’s 18 years and over, supporting rehabilitation, reablement and recovery within the comfort of the patients home.

We provide a 7 day service, between the hours of 8am - 8pm; therefore successful candidates will take part in a rota that covers the hours of the service, however we do offer a flexible and varied shift pattern. The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required.
