JOB SUMMARY

The Pharmacist appointed will be a member of the pharmacy team providing pharmaceutical services to the Southern Health and Social Services Trust. In order to meet the overall mission of the Trust the post holder will:

Assist the Trust Foundation Training Year (FTY) Lead Pharmacist with the delivery and quality control of the FTY programme within the Trust.

Act as an Educational Supervisor (ES) and provide support to others undertaking the role of Educational Supervisors.

Act as Designated Pharmacist Practitioner (DPP) for the FTY trainees and provide support to others undertaking the role of DPP.

Work as a Clinical Pharmacist providing a clinical pharmacy service to inpatient wards and clinics.

Work as a pharmacist independent prescriber within the Clinical Pharmacy Service.

Deputise for the Trust Foundation Training Year (FTY) Lead Pharmacist if required and when appropriate.

KEY DUTIES / RESPONSIBILITIES

Main Responsibilities

Assist the Foundation training Year (FTY) Lead Pharmacist with the delivery and evaluation of the foundation training year (FTY) programme within the Trust, in line with GPhC/PSNI standards and in collaboration with NICPLD.

Act as an Educational Supervisor (ES) and provide support to others undertaking the role of ES.

Act as a role model and leader for FTY trainees, promoting the role of the hospital pharmacist and demonstrate appropriate professional behaviours.

Able to set and maintain appropriate professional boundaries with FTY trainees.

Act as a Designated Pharmacist Practitioner (DPP) for FTY trainees and provide support to others undertaking the role of DPP.

Contribute to the teaching and assessment of clinical pharmacy skills that support professional practice, such as consultation skills, examination skills, medicines reconciliation, clinical reasoning, decision-making and prescribing.

Can competently teach, assess and monitor, as appropriate, the FTY trainee skills, knowledge, behaviours and understanding.

Engage with other stakeholders within the Trust to assist with delivery of FTY programme.

Provide support and train other pharmacists within the Trust about all aspects of the FTY programme.

Liaise with practice supervisors (PS) to evaluate the performance and monitor FTY trainee progress during hospital placement.

Raise issues and concerns about a FTY trainee pharmacists’ progress/capability/development with NICPLD as soon as issues arise.

Support FTY trainees in their learning.

Plan and organise own work according to agreed objectives and timeframes for the designated area.

Deputise for the Trust Foundation Training Year (FTY) Lead Pharmacist if required and when appropriate.

Provision of clinical pharmacy services

Be an active Pharmacist Independent Prescriber within the Clinical Pharmacy Service, registered with the Pharmaceutical Society of Northern Ireland and the Trust in line with local NMP policies.

Participate in the provision of a high quality clinical pharmacy service to patients to ensure the safe, effective and economic use of medicines.

Obtain a comprehensive and accurate medication history and complete medicines reconciliation on admission to ensure that patients receive the correct and appropriate medicine in a timely manner.

Manage patient’s own drugs to ensure effective prescribing and patient safety at all times.

Optimise patients’ medication on admission, during the inpatient stay and at discharge in accordance with patients’ diagnosis and assess treatment options available.

Review inpatient prescriptions to include identification of complex drug regimens, drug interactions, adverse drug reactions and rationalisation of medicines.

Develop good working relationships with the other members of the healthcare team representing the pharmacy profession and demonstrating professional accountability.

Negotiate and influence changes in prescribing practice to ensure that medication use is safe, effective and evidence based.

Proactively contribute to therapeutic decisions concerning patient management.

Provide high level of medicine and patient safety information to relevant stakeholders, as part of the multidisciplinary team using professional judgement to make clinical decisions where limited information is available.

Work in conjunction with the medical staff to prepare timely discharge prescriptions and prescribe discharge medications when necessary.

To complete medicines reconciliation for patients at the point of discharge to ensure an accurate and complete transfer of care is provided to primary healthcare professionals in a timely manner.

Provide advice and counselling to patients and their carers on the proper and informed use of medicines prior to discharge. This includes the provision of written or verbal information to patients with difficult behaviour or those with impaired cognitive function or impaired senses.

Ensure that delivery of pharmaceutical care to patients in the defined clinical area meets both national and local standards.

To reduce risk associated with medicines, report incidents and near misses in line with Trust policy and participate in incident investigations.

To participate in the supply and dispensing of pharmaceutical products and the provision of patient services.

Identify and respond to the specialist pharmaceutical information needs of patients, carers, doctors, nurses and other members of the healthcare team.

To ensure safe systems of working and compliance with written procedures.

To ensure that medicines are stored securely and appropriately at ward level in compliance with legislation and the Trust’s Medicines Code.

Assist in benchmarking the clinical pharmacy service and contribute to the development of performance indicators to monitor effectiveness and patient benefit.

COMMUNICATION AND INFORMATION MANAGEMENT

Communicate effectively with staff and maintain productive working relationships amongst your staff and with others.

To ensure the maintenance of appropriate documentation and records.

Report and record all accidents, untoward incidents, sickness and absence within your area of responsibility using appropriate forms.

Deal with and report complaints in keeping with Trust policy and assist with investigations when necessary.

QUALITY

To ensure that the needs of patients and their carers are at the core of the way that the Trust delivers services.

To ensure that the output and quality of the service is maintained in relation to short and longer term objectives.

To provide a service in accordance with published standards and local policies.

Work within the Trusts’ governance framework to ensure policies and procedures meet relevant statutory requirements, professional standards, policy guidance and workforce and regulatory requirements.

PEOPLE MANAGEMENT

Act as a role model and demonstrate the ability to provide safe, clinically effective and cost efficient use of medicines for pharmacy staff.

To motivate and maintain staff relationships and morale.

Report any signs of ill health/stress in colleagues.

Supervise and support the junior pharmacists and technical staff working within your area on a daily basis.

Utilise the skill set of the pharmacy team working in your designated clinical area and delegate tasks when appropriate.

TRAINING AND EDUCATION

Participate in the training of undergraduate MPharm university students.

Participate in the mentoring, teaching and training of pharmacists on the Foundation Programme.

Act as a supervisor to pharmacists undertaking the Independent Prescribing and/or Advanced Practice/MSc qualifications.

To participate in pharmacy education sessions.

To contribute to the education and training of medical, nursing staff and other healthcare workers.

Complete continuing professional development (CPD) in accordance with Pharmaceutical Society of Northern Ireland requirements and apply knowledge gained to current and future work as appropriate.

RESEARCH AND DEVELOPMENT

To collect and collate all relevant activity data for clinical service evaluation.

Maintain Good Clinical Practice training for Clinical Trials and participate in the dispensing of trial products when needed.

Undertake clinical audit and quality improvement projects in your designated clinical area.

Participate in research projects relevant to your clinical practice

Participate in the preparation of conference presentations, posters, writing reports and the writing up of publications.

ADDITIONAL RESPONSIBILITIES/ DUTIES

Comply with all Trust policies and procedures relevant to area of work.

Complete and maintain mandatory training requirements for your role.

Support the Trust in defending legal actions and participate in tribunals as required.

Acquire and/or retain the core technology skills required to support the use of digital healthcare systems e.g. Encompass.

To participate in the pharmacy dispensary rota.

Promote team working among other areas within pharmacy e.g. patient services, cancer services and procurement.

To ensure appropriate security measures are employed in the pharmacy department and in your designated clinical area.

To participate in the provision of other pharmaceutical services as required.

Support the Trust in its drive for seven day working in order to ensure it best meets the needs of our patients.

To participate in extended hours duties and bank holiday working as part of the pharmacy department rotas.

To participate in the weekend rota as per the needs of the pharmacy service.

To participate in the out-of-hours emergency on-call rota.

Any other duties as required in response to changing demands of the service.
