Job summary
The East Community Mental Health Team (CMHT) are looking for a skilled Senior Mental Health Practitioner to work within the service's Shared Care and duty team. The purpose of this is to provide consistent and pre-planned crisis care for patients within our caseload who are experiencing a decline in their mental health and require extra support.
The shared care team are part of the multi-disciplinary team and are centre to the daily functions of the CMHT. The role covers a wide range of interventions and communication with primary and secondary care is critical in making sure that the patients are at the centre.
Main duties of the job
- To participate in weekly team meetings, communicating service user needs andbe able to assist in prioritising the allocation of workload.To work in a focused way with patients to achieve goals identified by the patient.To contribute to the weekly multi-disciplinary meeting where service user'spresenting with an increased level of need are discussed and to present cases,on a rota basis, for team review.To formulate care plans in line with identified need and review in line with theTrust Care Programme Approach and Care Planning Policy.To provide clinical supervision and caseload management to support workers.To provide therapeutic interventions in line with the agreed care plan and riskassessment, utilising psycho-social interventions and CBT based treatmentTo assess risk, develop risk management plans, review and adjust in line with theservice user's needs.
