The team plays a key role in supporting improving patient flow, and ensuring high-quality medicines optimisation at the point of admission.

We are looking for a dynamic and confident technician who is passionate about clinical medicines management, enjoys working in a fast-paced environment, and can work autonomously while being part of a supportive multidisciplinary team.

The successful candidate will be registered with the GPhC and will hold an Accredited checking qualification. Applicants who are newly qualified will be offered a Band 4 initially then given the opportunity to automatically move to a Band 5 once all training and competencies are met.

We are looking for someone who:

  • Has strong medicines management experience, ideally in an acute setting.
  • Demonstrates excellent communication and organisational skills.
  • Can confidently prioritise and work independently in a busy environment.
  • Is adaptable, enthusiastic and committed to improving patient outcomes.

Hours of work will be 8:30 to 17:00 with the potential of rotational weekend and bank holiday working. Days of work will be discussed and agreed at interview.

  • Medicines clerking – accurately gathering and documenting patients’ medication histories.
  • Completing Anticholinergic Cognitive Burden (ACB) scores and escalating risks appropriately.
  • Ordering and supplying medications for SDEC/FIT patients in a timely manner.
  • Undertaking full medicines reconciliation, ensuring accuracy, compliance with policy, and appropriate clinical screening by a pharmacist.

  • TTA pack management and auditing, ensuring availability, rotation, and accurate documentation.
  • Dispensing and accuracy checking of own TTAs, supporting rapid discharge and optimised patient flow.
  • Managing SDEC/FIT stock levels, ensuring appropriate ordering, expiry checks, and governance across all areas.
  • You will work closely with pharmacists, medical staff, nursing teams, and other allied health professionals to deliver a safe, responsive, and efficient service.
