Job summary

Main duties of the job

The post holder will carry out individual assessments of health needs, implement and evaluate packages of care as part of the Healthy Child Programme, at Community, Universal, Universal Partnership and Universal Partnership plus level, with a focus on Universal Provision and Early Help and support.
