This is an exciting opportunity to lead, facilitate and deliver improved pathways for patients with long term conditions and cancer at the end of life stages across Worcestershire Acute Hospitals NHS Trust.

Working as a member of a hospital based specialist palliative care team, as part of the wider primary and secondary multi-disciplinary teams and the independent sector to ensure a cohesive, holistic and research based approach to the End of life care service. The overall outcome of this post is to make quality improvements in the provision of end of life care within Worcestershire, by introducing End of Life care initiatives described by the National End of Life care strategy.

Provision ofsupport & development of a comprehensive highly specialist service to patients with long term conditions and cancer (and their carers/family) at the end of life stages.

This is an opportunity for thepost holder to provide clinical leadership within this area of care and lead, develop and implement specialist education, training, research and audit

The post holder will work within the 7 day rota, to provide BH and Weekend CNS service.
