CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!  We have an exciting opportunity for a Band 6 Early Intervention Vehicle (EIV) Practitioner to join our service on a 10 month fixed-term, part time basis (29 hours per week).   The Early Intervention Vehicle (EIV) pathway delivers timely crisis management for patients living in the Herts Valleys area, with the aim of preventing avoidable hospital admissions. Practitioners provide comprehensive clinical assessment, delivered jointly by a nurse or paramedic alongside a therapist, supporting patients in their own homes.   The EIV pathway operates 7 days a week, currently between 08:00 and 18:30 (subject to change as the service develops). The EIV pathway works in close alignment with the Unplanned Care service, which provides high quality, clinical care in patients’ homes during times of crisis.

The newly established EIV service supplements and supports delivery of both Rapid Response and Discharge to Assess in the Unplanned Care service.   As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

  • Deliver high quality, evidence based clinical care to patients across a range of community settings.
  • Undertake comprehensive risk assessments to determine patients’ ability to remain safely at home, coordinating appropriate nursing care and support packages where required.
  • Identify and respond to safeguarding concerns, managing issues directly where appropriate and making timely referrals for investigation in line with safeguarding policies and procedures.
  • Provide autonomous clinical care to patients with undifferentiated, complex, and long term health needs.
  • Work within Clinical Guidelines, Policies, Procedures and Patient Group Directions (PGD's) utilising expert judgment to work outside these frameworks where necessary. Practice in accordance with clinical guidelines, policies, procedures, and Patient Group Directions (PGDs), applying professional judgement to work outside these frameworks where necessary.
