The Bradford EIP service has an exciting opportunity for a Band 6 Mental health Nurse, Social Worker, or Occupational Therapist to join the EIP Assessment team as a Care Coordinator/Care Navigator.

The Early Intervention assessment team is a dynamic high performing service based in Bradford. The successful applicant will join an experienced, established team of supportive specialist mental health professionals and will work closely within a MDT framework.

The role focuses on specialist assessment of individuals presenting with potential first episode psychosis and those at risk of developing psychosis (At Risk Mental States) across the Bradford region. The successful postholder will undertake care coordination responsibilities working closely with individuals referred, their carers and support networks, offering supportive interventions during the period of care.

The BDCT EIP service is committed to supporting staff with regular clinical and caseload management supervision and offers a culture of learning and ongoing progression to help you grow. In line with National EIP safer staffing practices, staff caseloads are protected to enable effective, responsive high quality work.

Main duties of the job

Accountable for the triage, assessment and formulation of new referrals to the EIP team. Signposting to the longer-term part of the EIP service or to other services, where appropriate.

Co-ordination of care plans for people who are in the early stages of psychotic illness between the ages of 14-65.

Assess health and social care needs and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty.

Deliver and administrate care plans in partnership with multi-disciplinary colleagues and a range of health and social care agencies, within the CPA structure.

Engage service users and carers assertively and offer care/interventions within the least restrictive setting in normal community environments.

To provide an age, culture and gender sensitive service.

To ensure Bradford and Airedale Child Protection Guidelines and procedures are fully understood by self and others. To ensure the safety needs of children are met at all times and take appropriate action when a child is felt to be at risk.
