Job summary

Main duties of the job

The successful candidate will be a senior clinical nurse within the Enhanced Care Home Support Service, leading a team and providing both an urgent, reactive response to people cared for within the nursing and residential homes in Warrington. To ensure a timely clinical assessment, diagnosis and treatment as required, as well as proactive, planned holistic assessments, aimed at optimising management of medical conditions, including frailty, and supporting with Advance Care Planning. The aim is to support people to remain within the care home setting rather than admit to hospital, where appropriate.

The role will require the postholder to act as an independent autonomous practitioner to provide treatment and advice to patients and care home staff, both face to face and over the telephone, which will contribute to the assessment, diagnosis, care and treatment of patients, preventing unnecessary hospital admissions.

The postholder must possess and demonstrate extended clinical skills in the assessment, diagnosis, treatment and management of a wide range of patient care needs, enabling professional autonomy and clinical judgement in managing and referring patients on to wider services, whilst maintaining close working relationships with GPs / Primary Care Networks to support the Enhanced Health in Care Homes Framework and Multi-Disciplinary Team (MDT) meetings.
