
We are seeking a Locality Discharge Liaison Practitioner to support safe, timely discharge from inpatient mental health services into the East communities of Louth, Skegness and Boston. Working across wards, LMHTs, social care and wider partners, the post holder will coordinate discharge planning, assess ongoing service needs and support smooth transitions to appropriate services. The role plays a key part in reducing delayed discharges, improving patient flow and promoting person centred, recovery focused care across the locality.
