We are seeking to recruit a care coordinator for Oldham Early Intervention Team.
As a Care Coordinator, you will work with service users identified as having a first episode psychosis, being in the early phase of a psychotic illness or at risk mental state. You will assess the needs of the service users and deliver interventions in line with evidence based practice and NICE guidance. You will provide advice and supervision as required to other mental health professionals within the multi-disciplinary team and contribute to ongoing service developments.
Main duties of the job
Clinical
- To act as a Care Coordinator carrying a complex caseload, meeting the criteria for the service delivering comprehensive support for identified needs.
- To identify the needs of service users by utilising Trust approved research validated assessment tools.
- To employ an assertive outreach and flexible approach as and when necessary to increase levels of engagement.
- To liaise with all appropriate statutory and non-statutory agencies as required in relation to the service user’s needs.
- To implement and manage co working strategies as necessary within the multi-disciplinary approach
- To carry out interventions in line with the psychosocial approach of the overall service focusing on recovery and re: enablement.
- To ensure carer assessments are carried out and identified areas of need are addressed.
- Engage in family work where needed and provide psycho social education and information as required.
- To carry out physical health screening and intervention in line with NICE guidance (training provided)
- To identify risk issues and implement and evaluate risk management plans according to policy and procedure.
- To be accountable for ensuring that within the resources at your disposal all service users receive a high quality service.
- Meet Trust standard CPA requirements Ability to formulate and deliver care to complex clients and their families.
