The Discharge Lounge has significantly improved hospital efficiency by offering discharged patients a space to await transport and receiving take-home medication promptly. Data indicates an enhanced patient experience, attributed to quicker medication processing, earlier transport availability, and exceptional care from our dedicated team.
Support Worker responsibilities include coordinating with wards to bring patients to the lounge, caring for those awaiting discharge, managing transport bookings, facilitating communication with families and care providers for discharge arrangements. The role involves driving the hospital trust vehicle, acting as an escort for patients with additional needs and handling equipment and medication deliveries.
Additionally, the post holder will assist the Site Manager, participate in patient transfers from wards and the Emergency Department (ED) and providing HCA support, within the limitations of the role, to all emergency and inpatient areas dependent on demand. The position requires working unsociable hours, encompassing a full rotation of days, nights, and weekend shifts.
Hours per week: 37.5
The Discharge Lounge has a small team of substantive staff, but they are required to communicate widely with staff, services, and external providers to the hospital; therefore, excellent communication skills are an essential element of the job.
Liaise closely with hospital wards to bring patients to the Discharge Lounge, complete relevant SBAR for the safe transfer of the patient.
Provide assistance and support to patients awaiting discharge, including nourishment and care needs These would include continence care and supporting patients to the toilet, pressure area care, use of oxygen, taking observations
Responsible for problem solving and proactively working with the MDT to manage and facilitate the booking of transport for discharged patients.
Working flexibly and proactively to ensure the most efficient use of all available means of transport to discharge patients safely and comfortably.
Communicate effectively with families, care providers and discharge coordinators regarding discharge arrangements
Ensure clear and concise communication with relevant hospital staff, including ward staff and site team.
Collaborate with the Discharge Lounge team, site manager and discharge coordinators to ensure a smooth and efficient patient discharge process.
Ensure appropriate escalation of any issues to the site manager in a timely manner, preventing delayed discharges.
