The role of Discharge coordinator is an integral part of the MDT providing support and specialist advice to the clinical nursing team, AHPs, medical teams and extended MDT including social care and at times outside agencies. You will be expected to provide the highest level of support and advice to enable the timely discharge of vulnerable and graduate premature infants to home and to the care of their family/carers and evaluating our services.
The post holder will use their experience and knowledge to support transition from hospital to home, working closely with community partners ensuring a safe and supportive environment is achieved with ongoing and continued support is available where necessary.
You will also be expected to actively contribute to multi professional and multi agency meetings being able to give reports, information and updates to the relevant parties including safeguarding meetings for which there is a link role between the safeguarding team and the neonatal unit. Safeguarding updates and pathways already in place within the trust will need to be imbedded in daily practice with focus on pathways particular to the neonatal ward to be strengthened. The post holder will be key for leading on quality and safety initiatives which will include the setting and monitoring of standards with the direction and support of the Matron.
We have an opportunity for a Band 7 Neonatal Discharge Coordinator at 0.5 WTE which has become available in Neonatal ICU , Royal London Hospital to join our dynamic and friendly team. We are looking for a motivated , ambitious, and experienced Neonatal Nurse who will support and work closely with the Neonatal team , working in partnership with families in the unit to ensure seamless transition of care from hospital to community by liaising with the Neonatal Community outreach team , pertinent agencies within and outside agencies within and outside the organisation.
