Are you experienced, caring, compassionate and enthusiastic? An exciting opportunity has arisen for a Discharge Co-ordinator to join our friendly, supportive team on a full-time basis.

The Discharge Co-ordinator works as part of a multi-disciplinary team supporting patients through the admission pathway to successful discharge form hospital. You will Collaboratively work with services users, families/cares, the wider MDT and partner organisations.

We are keen on attracting people who have a desire to work with Older Adults (65 years +) with mental health problems and complex needs. It is essential that candidates have the ability to use their own initiative, have good communication and can manage their time effectively.

Main duties of the job

The candidate will:

Work as part of Ward 19, a male and female split ward for Older Adults (65 years and over) with complex mental health conditions and support the multi-disciplinary team, contributing to the safe care provided for patients in an in-patient setting.

Contribute to the admission and discharge process for patients and their families/carers.

Enhancing the admission and discharge process by supporting the clinical team, families and carers ensuring a smooth transition throughout the elements of relevant care services.

Work in close liaison with the community/crisis teams, key clinician/MDT in ensuring an efficient admission process and effective discharge planning and care.
