We have an exciting opportunity for a Specialist Diabetes Dietitian to join the multidisciplinary Hounslow Community Diabetes Service (HCDS), based at Heart of Hounslow Centre for Health. This service is commissioned to provide a specialist integrated diabetes services to adults with Type 2 diabetes in the London Borough of Hounslow, with the specialist dietetics team delivering X-PERT structured diabetes education and specialists diabetes clinics. The service is delivered at sites across the London Borough of Hounslow and incorporates a mix of face to face and virtual patient sessions, which can include evening or weekend shifts. The model of service delivery is reviewed regularly to best meet the needs of the patient population.
The candidate should be able to demonstrate advanced knowledge and experience of diabetes clinical management. We are also looking for a Dietitian who has experience of delivering diabetes structured group education sessions e.g X-PERT Diabetes; X-PERT Insulin
You will receive support from the Lead Dietitian in the team to help you to further develop your clinical and professional skills. Your career progression is important to us, and we are committed to continuous professional development. There is dedicated administrative support and IT facilities across the Trust. We are looking for candidates who are enthusiastic and adaptable to change, with excellent clinical skills in Type 2 diabetes management and competence in the use of IT systems. Candidates should exhibit good organizational skills. You will require highly developed interpersonal skills to establish and foster strong relationships with a range of key stakeholders in delivering care. This role will require travelling to community venues, you must therefore have access to your own transport and the ability to transport educational resources, as required.
You will have proven experience of working with different patient groups and those with diabetes across a culturally diverse population. You will demonstrate motivational skills to be able to provide health advice to patients based on their individual dietary needs, preferences, and restrictions.
