An exciting opportunity has arisen for a enthusiastic, motivated and caring assistant practitioner to join our community Dietetic team.
You will work as part of a wider community rehabilitation team to deliver assessment, treatment and nutritional care planning for Calderdale residents either within their own home or from a clinic setting.
As a dynamic evidenced based service, the Calderdale Dietetic team is heavily involved in transformative service change, research and development, to ensure that high quality standards are held and maintained.

Main duties of the job
Activities will include
- Assessing and implementing treatment to scope of practice.
- Monitoring, evaluating and adjusting treatment within the scope of practice.
- Assisting registered practitioners with complex treatment.
- Supporting patients with enteral feeding regimes and adjusting care plans within scope of practice.
- Ordering oral nutritional supplements, enteral feeds and other home enteral nutrition equipment.
- Liaising with other agencies, including care providers, District Nurses and Nutricia Nurses.
- Checking of blood test and swab results, reporting abnormalities to registered staff, and requesting treatments.
- Managing queries from patients, carers and other health professionals relating to patient care.
- May include leading and assisting with group advice / education sessions
- Maintaining accurate electronic patient records.
- May take the responsibility for the training of others, including delivering MUST training.
- May involve delegating tasks to rehabilitation assistants.
- May involve supervising the routine tasks of others.
