An exciting opportunity has arisen for an enthusiastic and motivated Band 3 Dietetic Support Worker to join our Integrated Nutrition and Dietetic service at Stockport NHS Foundation Trust. This is based in our community team at Kingsgate House. The team comprises of an Integrated Nutrition and Dietetic Service Lead, Team leader, band 7, band 6 dietitians, Dietetic Support Worker and Secretary. We are integrated service with the Acute dietetic service at Stepping Hill Hospital and patient education colleagues.
The role will be focused on providing a high standard of dietetic care to community patients requiring nutritional support. There will be opportunities working alongside dietitians in their specialist areas.
As a Support Worker you will be required to complete the Care Certificate programme. Professional development is encouraged through supervision, appraisal, mandatory training and CPD activities. Supervision is provided by senior clinical staff.
You will require to have access to a vehicle which can be used for work purposes or will be able to travel as required to meet the needs of the service. The Integrated Nutrition and Dietetic Service is based across acute and community settings. This primarily based in the community in Stockport. The Dietetic Support Worker assists the Dietitians in certain aspects of this work including: Liaising with the Dietitians and following plans of care for patient follow up Actively participate within group education under the supervision of the Dietitian. Data entry regarding product orders Assisting with the daily supply of dietetic products to patients in their own homes and working with the enteral feeding companies A working knowledge of different products and their clinical use is required.
Assisting selected patients on the wards to choose preferred and clinically suitable food and drinks, under the supervision of a Dietitian. Responsible for the service ordering of supplies and consumables. Telephone calls to nursing homes regarding residents Electronic ordering of products
Main Duties and Responsibilities To deal courteously and efficiently with incoming telephone calls from healthcare professionals, patients and members of the public. Liaise, where necessary, with Dietitians or action as appropriate to role Office administration. Provide cover for some secretarial duties during leave Assist with the supply of therapeutic dietary products and equipment to the service. Check stock deliveries against delivery notes where required. Assist the Dietitians with monitoring of product supplier company performance against contract. Assist Dietitians with the arrangements for patients on tube feeds. This may include completing computerised registration for the home feed service, telephone calls and emails to GP surgeries, liaison with acute Dietitians and the home feed service company. Enter data as needed into tables. . Provide follow-up support to selected patients who have been seen first by the Dietitian and monitor nutritional care plan this may be in clinics, patient’s own homes or nursing homes. Gather information for referrals made to the Dietitian Visit patient’s homes to undertake weight assessments Actively participate within group education under the supervision of the Dietitian Attend Trust mandatory training. Attend and participate in meetings as required Provide training on malnutrition universal screening tool/ nutritional screeningwhere required. Support nutritional analysis using computer software, where required Adhere to the Health and Care Professions Council (HCPC) and British Dietetic Association (BDA) 4 Responsibility for Policy and Service Development and Implementation Communicate to the Dietitians issues requiring further development. Assist in the development of patient advisory literature. Assist in quality assurance for care and delivery of service to the Trust. Propose changes in working practices or procedures for own area. Participate in quality improvement initiatives within the department Responsibilities for Human Resources and Leadership To work as an active, efficient and supportive member of the Nutrition and Dietetic Service staff across several integrated teams. Responsibilities for Teaching and Training Assist in the development and delivery of education and training programmes. To support new/inexperienced staff members by participating in their induction programme/ training as required. Support the training of Dietitian students where appropriate Responsibilities for data and information resources Accurately record statistics on activity and audit in compliance with Trust and department requirements if required. Complete nutritional analysis of recipes and menus as directed by senior Dietitian Research, Development and Audit Participate in departmental audit as required Participate in relevant policy development as required Physical Skills and Effort Manoeuvre goods and equipment and assist people to move, in accordance with manual handling regulations and good practice. Standard keyboard skills required The role requires physical effort to carry scales and hoist scales to homes and use with patients. This involves working within spaces in the patient home Anthropometric measurements may need to be undertaken following instruction from the Dietitian The role will cover the region and requires movement between locations General Duties for all employees 5 Hand Hygiene To assist the Trust in reducing healthcare acquired infections (HCAI’s) all staff should be familiar with all the Trust’s Infection Prevention policies which are appropriate to their role. You are required to attend mandatory training in Infection Prevention and be compliant with all measures known to be effective in reducing HCAI’s” Safeguarding All Stockport Foundation Trust employees are required to act in such a way that at all times safeguards and promotes the health and well-being of children, young people and vulnerable adults. Familiarisation with and adherence to the policies and procedures relating to child protection and safeguarding of children, young people and vulnerable adults are adhered to. Prevention To actively work with patients/clients/service users to identify appropriate opportunities in Making Every Contact Count for preventative interventions which may reduce the risk of future harm to health and wellbeing and to provide brief advice and refer or signpost to sources of further information and support which may include advice on lifestyle behaviour and social care needs as well as safety and management of long term conditions. Data Protection, Confidentiality and Information Governance The post holder must abide by all relevant Trust and departmental policies including information governance, confidentiality and data protection and, undertake the annual data security awareness mandatory training. The post holder is reminded that any breach of the Trust’s information governance and security policies and procedures, including data protection legislation, will result in disciplinary action. Data Protection Legislation – the post holder is required to process all personal data relating to patients and staff, whether in paper, electronic or other media, in accordance with the Data Protection Act 2018 and the General Data Protection Regulations (GDPR), ensuring the security and confidentiality of data at all times. The post holder must not for their own benefit or gain, or to divulge to any persons, firm or other organisation whatsoever, any confidential information belonging to the Trust or relating to the Trust’s affairs or dealings which may come to their knowledge during employment. Health & Safety Under the Health and Safety at Work Act 1974, the Trust has a statutory responsibility to provide and maintain a healthy and safe environment for its staff to work in. Equally the post holder is required to work within the Trust Health & Safety Policy and fulfil a proactive role towards the management and control of risk. This entails the identification, assessment and the immediate reporting, using the Trust 6 Incident Reporting System, any incident, accident, hazard or near miss involving patients, service users, carers, staff, contractors or members of the public.
