Diabetes specialist nurse plays a crucial leadership role in diabetes care, balancing expertise with management and service development responsibilities. Main duties are providing a high quality care diabetes service, providing specialist care and treatment programmes for patients , families and carers. Assessing plans and evaluating effectivnessof diabetes programmes provided by the nursing team to ensure high standard.
Responsible for the day to day leadership of the specialist nurses and facilitating the development of the specialist nursing service. Ensuring that they are receiving high quality diabetic care as well as ensuring that their medications are appropriate and patient centred to achieve good outcomes and reduce long term effects of diabetes.
Supporting patients with the available technology eg CGM, Flash glucose monitoring and insulin pumps.
Empowering patients to take control of their diabetes and promote self management. Influencing
practice and partnership working across primary and secondary care.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
