Job overview

We are looking for a band 7 registered nurse with a special interest in diabetes with drive, enthusiasm and vision.

As a Community Diabetes Specialist Nurse you will play a key role in delivering Diabetes Specialist Services to patients. You will also enable and educate GP’s, Practice Nurses and Community Health professionals in supporting their patients with diabetes in their usual care setting. The community diabetes service also provides structured education for people living with type diabetes.

You will have excellent interpersonal and communication skills and will be expected to ensure productive working relationships across acute, community and social care.

The role will enable you to develop leadership experience as well as advanced clinical skills. You will be working in the London borough of Ealing in different locations and will also be required to do home visits.

As a Nurse with active NMC registration and a recognised diabetes qualification you will demonstrate strong decision making skills with the ability to motivate other non-specialist practitioners into improving their knowledge in the management of the person with diabetes.

Main duties of the job

The role focuses on evidence-based diabetes care, collaborating with multidisciplinary teams, and managing a caseload.

Responsibilities include patient assessment, care planning, leading clinics, home visits, clinical procedures (e.g., insulin adjustments), and supporting education strategies.

The aim is to upskill GPs and nurses, work across organizational barriers, and contribute to developing protocols for diabetes care.

Communication is vital, involving the exchange of complex information for collaborative outcomes.

The role includes mentoring students across medical disciplines, fostering their education, and ensuring high standards of care, all in line with NMC and Trust policies.
