As a Registered Nurse, the post holder is required to adhere to NMC standards and guidelines of practice, education and conduct.
The role involves working as an autonomous expert Diabetes practitioner delivering evidence–based care and structured education to people with diabetes, their families and other health colleagues in a community setting. This includes participating in joint clinics & domiciliary visits for patients with diabetes including those living in Care Homes, who have complex needs whilst educating primary care staff to enhance their skills to provide an excellent diabetes service.
The Diabetes Specialist Nurse will be expected to teach and support programmes of care to individuals, primary care professionals and act as a clinical resource for community nurses.
The post holder will have specialist knowledge and skills in an area of Diabetes Specialist Nursing. The post holder will be expected to be a resource for other members of the team; support clinics across the locality as necessary and participate in clinical supervision.
The Diabetes Specialist Nurse will work with colleagues in primary and secondary care to develop an integrated care delivery system ensuring that this is based on research based practice.
The Diabetes Specialist Nurse will be expected to teach and support programmes of care to individuals, primary care professionals and act as a clinical resource for community nurses.
To provide structured patient education for people with Type 2 diabetes to empower them in their self-management of their diabetes, this could be face to face or virtually using Microsoft Teams.
To assist and lead in educating health care professionals in both primary care and community nursing teams. .
To Provide joint diabetes clinics for primary care (when competent) to assist and support them to manage their patients and imparting knowledge in order to upskill them.
Work autonomously to carry out individual patient reviews and give clinical advice to both patient and healthcare workers.
Act as an expert resource for evidence-based care within the SCFT community and Primary care setting.
To participate in team meetings and assist in agenda and minute documentation
To support the senior DSN team to compile and update clinical audits to improve the service.
To maintain competence in specialist area of practice through clinical supervision, evidence based journal reviews and attendance of on courses and conferences.
To adhere to keeping updated on Policy updates and trust training requirements.
