Working within a multidisciplinary diabetes team, the Community Diabetes Pharmacist will collaborate with nurses, dietitians, consultants, GPs, and other health professionals to deliver integrated care closer to home. The role also contributes to clinical service delivery, patient education, quality improvement, service development, and workforce training, supporting CLCH’s commitment to high standards of governance and population health management.

This post offers an excellent opportunity to develop expertise in community diabetes care and broaden experience across healthcare settings, with support for ongoing professional development.

  • The post holder will manage medicines-related risks, contribute to clinical governance, audits, quality improvement, and service development, and support the education and training of colleagues and learners.
  • You will maintain accurate clinical documentation and continue to develop professionally through CPD in line with GPhC and Trust standards.
