NHS Fife Older Adult Community Mental Health Services.

A vacancy has arisen within the Older Adult Community Mental Health Team, and we would like to welcome applications from highly motivated individuals, with a background in working with older adults with Dementia, to join us as a Dementia Link worker. You will be based at Queen Margaret Hospital, Dunfermline.

We are seeking a confident individual who has the ability to work independently but also as a key member of our multidisciplinary service, within our wider Older Adult Community Mental Health Team. You will hold your own caseload and be expected to deliver a minimum of one year Post Diagnostic Support to people newly diagnosed with dementia. The Link Worker will provide advice, information and support to people with dementia and their carers who are in the process of, or have been diagnosed with dementia. The link worker will be using the 5 Pillar Model of Post Diagnostic Support.

The post holder will demonstrate good awareness of local services and resources for optimising care provision and patient experience.

You must possess excellent communication skills and be adaptable to change, and have experience of working with older adults.

There is a need to use IT systems such as Microsoft Office and Morse

Successful applicants will receive a full induction, and supervision, and there will be an expectation that staff will either have, or work towards, SVQ level 3 in Health and Social Care.

IT IS VITAL THAT THE SUCCESSFUL CANDIDATE CAN TRAVEL ACROSS A LARGE GEOGRAPHICAL AREA IN A TIMELY MANNER

Working pattern – The service operates within office hours Monday to Friday 0900-1700.
