Job summary

The county of Herefordshire is a classic mix of rural and city living and can provide an excellent work-life balance. We are looking to recruit a dynamic Consultant to provide senior medical advice, support and clinical leadership in the Hereford City Neighborhood Mental Health Team. You will be joining a friendly and close-knit consultant body in the county.

Herefordshire & Worcestershire offer excellent living and working prospects. Main duties of the job

The Team (NMHT) underwent Community Mental Health Transformation in 2021, following the principles outlined in the NHS Community Mental Health Framework (2019). Our way of working aims to improve access, reduce fragmentation, and help people maintain wellbeing, stability, and meaningful lives within their own communities.

The team provides accessible, person-centred mental health care supported by local Primary Care Networks. Its purpose is to offer holistic, trauma-informed support for adults with a range of mental health needs, including severe, complex and enduring conditions. The team aims to meet the needs of both people with Severe Mental Illness (SMI) as well as those with trauma histories, personality difficulties, co-morbid substance use, long-term physical health conditions, and social adversity. To this end, the team works collaboratively with primary care, social care, voluntary sector partners and community services to deliver integrated, recovery-focused care. The aim is seamless care across organisational boundaries with a no wrong front door referral policy and a step-up and step-down flow of patient care between primary and secondary services.

The team is working to deliver access to HMGNMHT services within 28 days of referral, and are currently meeting this target.

There is a weekly referral meeting where new referrals are discussed together with cases of concern, and a monthly team meeting for education and team administration.

Job responsibilities

The Consultant Psychiatrist has authority and responsibility as agreed with their supervising Clinical Director.

Specifically this will include:

Providing direct psychiatric services through the comprehensive assessment, diagnosis, care plan and treatment formulation of patients. Provide guidance on evidence-based treatment and effectiveness and addressing patients biopsychosocial needs. Also continuously evaluating and acting on outcomes.

Responsibility for organising and prioritising own and others workload in the day-to-day allocation of work.

Acting as an autonomous, registered practitioner who is legally and professionally accountable guided by the professional code of conduct and Trust guidelines and protocols. Maintaining organisational knowledge relating to Trust protocols and procedures.

Working within appropriate governance frameworks in the area of the Mental Health Act (including Community Treatment Orders), Mental Capacity, choice and control and the safety of patients, carers, staff and the general community. Mental Health Act Work as appropriate.

Effective risk management at team level by accident/incident reporting, assessing, and controlling risk.

Initiating referrals to other health professional specialist services and agencies.

Providing patients with information and education thus ensuring they have meaningful choices that promote dignity, independence, and quality of life.

Liaison with families and carers wherever possible (with the patients consent) in treatment planning.

Maintaining good medical practice standards for documentation and clinical record keeping.

Ensuring practice is supported by research, evidence-based practice, literature, and peer review.

Effectively communicating with colleagues, peers, senior managers and Clinical leads within the Trust.

Flexibility to shape and respond to changing service requirements.

The post is 1.0 FTE and amounts to 10 programmed activities. Flexible working arrangements can be accommodated, pending service requirements being met. Following appointment there will be a meeting at no later than 3 months with the Clinical Director to review and revise the Job Plan and objectives of the post holder. The overall split of the programmed activities is 7.5 PAs to be devoted to Direct Clinical Care (DCC) and 2.5 PAs to Supporting Professional Activities, as outlined in the timetable below.
