Wye Valley NHS Trust and St Michael’s Hospice, in the beautiful county of Herefordshire, are looking to appoint to a new consultant post in Palliative medicine as a result of service expansion . The team provides medical leadership, direct patient care, training, teaching and opportunities for quality improvement and service development across WVT acute and community hospitals, Herefordshire and mid Powys community and St Michael’s Hospice.

The post holder will act as Lead for COMPASS (Community Outreach Model for Palliative Assessment, Support and Step up/down) based within Hereford County Hospital and the Virtual Wards from Nelson House

This post holder will work with an established specialist palliative care team including consultants, CNSs and End of Life Care Facilitators. This post will provide consults in acute receiving areas and review new and existing patients in the virtual ward service including domiciliary visits, board rounds and MDT discussions.

There is a wealth of opportunity for palliative medicine training and multi-professional teaching supported by facilities at St Michaels Hospice and WVT including a simulation suite.

The post is 10 PAs with  2.5 SPAs. See the Job Description for proposed job plan.

The post includes 1 in 5 second on call.

Main duties of the job

The post holder will clinically lead the community outreach model service (COMPASS), providing excellent palliative care in acute receiving areas and upskilling generalist colleagues.

The post holder will participate in the weekly Herefordshire SPC service wide MDT and HCH SPC weekly MDT of patients known to the hospital team.

The post holder will review new referrals and existing patients in the virtual ward service including undertaking domiciliary visits, board rounds and MDT discussions.

The post holder will work with the 3 other senior palliative medicine colleagues to foster a collaborative, team approach that nurtures a seamless SPC service across organisational boundaries and in turn, integrates with non-SPC colleagues and teams covering all places of care in Herefordshire and mid Powys.

The post holder will be available for all SPC professionals caring for patients across the localities, specifically focused on the acute and community environment.

The post holder will participate in cross cover with consultant colleagues including clinical advice and 2nd on call for sickness and annual leave.
