Job overview

The post holder would be expected to champion a frailty approach to care and use comprehensive geriatric assessment to achieve the best care possible outcomes for older adults living with frailty in the Trust.

They would be responsible for the care of 12-14 patients on a 25 bedded, acute delirium and dementia ward. This ward is staffed with a variety of trainee Doctors, and it is expected that clinical supervision would be provided to some of these trainees. It is expected they would lead the ward-based MDT.

The successful applicant would spend some clinical sessions with the Acute Frailty Team providing clinical supervision to the MDT and the advanced clinical practitioners. They are expected to work with the team in ED and on the MAU to complete acute comprehensive geriatric assessment for older adult’s presenting with frailty crises and provide senior level reviews.

There will be the opportunity to develop a weekly clinic, which will be tailored to the successful candidates interests.

Hours per week: 10 PAs

Main duties of the job

The successful candidate is expected to participate in the unselected acute medical take rota.

The successful candidate will be also be expected to work flexibly and achieve high quality care with particular emphasis on:

implementing and achieving the targets set by NHS England for older people and frailty care and helping achieve the targets set by GIRFT

ensuring NHS Standards for Out-Patient waiting times are achieved

ensuring that optimal use is made of all resources allocated to the post

developing appropriate services and techniques required to meet clinical needs within available resources.

working together with colleagues to ensure effective management of inpatients, internal referrals, outpatient clinics and investigational sessions

working together with primary care colleagues to ensure effective management of community patients

participation in the education and training of medical students, doctors, and professions allied to medicine including those in primary care

demonstration of a firm commitment to the principles of clinical governance

developing and maintaining robust systems for effective clinical audit in the speciality and general medicine, as appropriate

participate in developing services as indicated by recommendations of the Royal College of Physicians, NICE and specialist societies about development of a Perioperative service unit.
