Main duties of the job
The objective of the COMPASS service is to enhance the Specialist Palliative Care (SPC) workforce to improve early identification of patients in their last year of life at the point of admission, reduce unnecessary hospital admissions, and shorten the length of stay for GSF (Gold Standards Framework) patients. This role will also provide support in the community through the Single Point of Access (SPoA), including care homes and patients receiving Hospital at Home (H@H) services as required, with the opportunity to work alongside Health & Care Trust teams. The intention is to leverage Specialist Palliative Care expertise to benefit generalist patients, while ensuring that patients requiring specialist level care continue to be supported within existing commissioned SPC services.
Key components include Integration with the Hospital at Home model and other community pathways to enable step-up and step-down care development.
