We are looking to recruit an enthusiastic,  professional, and compassionate Band 3 Community Support Worker to join our community team.

This post is not open to trainees therefore the successful candidate should have completed the Diploma in Health and Social Care Level 3 or equivalent.  Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).

The successful candidate will join our specialist multi-disciplinary team whose aim is to provide holistic, person-centred care to older people who are experiencing a range of mental health conditions.  We work alongside  people  in care homes and in their own homes.

We work closely with families/carers and other professional disciplines to ensure a comprehensive and valuable experience is had by all involved.  We pride ourselves on being leaders in delivering high quality compassionate care whilst protecting and promoting staff wellbeing and development.

ICLS team covers the Teesside area and so the successful applicant will need a full drivers' licence and vehicle access to carry out their role.

Detailed job description and main responsibilities

The post holder will support registered members of the team to deliver evidenced based interventions to help older people with their mental health using  interventions from our Pathways of care.

The post holder should also be expected to carry out physical health monitoring to help support older people.
