Job overview

We are looking for a Band 3 Stroke Rehabilitation Assistant with relevant experience delivering rehabilitation programmes,  to work collaboratively within the Community Stroke Service across the Scarborough and Ryedale locality.

We provide assessment and rehabilitation within the community setting for adults, from 18 years upwards who have had a Stroke. We work closely with professionals, services and agencies across the whole Stroke pathway.

You would be part of a small, dedicated, knowledgeable multi-disciplinary stroke team. You will have regular supervision and training opportunities and be supported in your development by experienced staff members. You will be joining the team at an exciting time of service development towards implementing the National Service Model for an Integrated Community Stroke Service, with a focus on creating a robust early supported stroke discharge service. You will have the opportunity to develop clinical skills and knowledge, operational awareness, and contribute to service improvement.

Main duties of the job

You will be expected to support with the provision of a seamless Stroke service from the local acute hospitals into the community. You will be carrying out treatment programmes with our community stroke patients which have been created by qualified therapists (Speech and Language Therapists, Occupational Therapists, Physiotherapists). Your role will include ordering and provision of equipment as directed by our Band 4 Assistant Practitioner and qualified therapists. You will support with duties including booking and arranging appointments, assisting with data collection, and ordering of supplies for the service.

Applicants should have a strong value base and promote holistic person-centred care. Previous experience of working within Stroke services and community experience would be advantageous. Additional specific training can be provided.

The post holder needs to be flexible, enthusiastic, have good time management skills, confident IT skills, and work well in a team as well as independently.
