We are looking to recruit a NMC registered nurse to work within both Rapid Response  and Core Nursing team in Chesterfield and North East Derbyshire.

Rapid Response  Service working hours 8-1830 daily 7 days a week.

Core service hours 08:30-16:30 variable hours 7 days per week

Main duties of the job

Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.

Within Rapids we have 3 bases across our locality - Clay Cross Hospital, Walton hospital and ARC at Clowne. Their is an expectation to travel across the locality as and when required to meet the needs of our patient group.

Core Services are based locally within each area covering Chesterfield and North East Derbyshire

Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient’s privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment.

Implement / refer on to the relevant services as appropriate.

Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.
