We are seeking a highly motivated and compassionate Band 5 Community Staff Nurse to join our dynamic and supportive District Nursing team. This is an exciting opportunity to provide high-quality, patient-centred nursing care within the homes of individuals across CTMUHB.
As a key member of our team, you will be responsible for assessing, planning, implementing, and evaluating holistic nursing care for a diverse caseload of patients, promoting independence and well-being. You will work autonomously, managing your own caseload, whilst also collaborating closely with a multidisciplinary team including GPs, social workers, therapists, and other healthcare professionals.
This role requires excellent clinical skills, strong communication abilities, and a commitment to delivering empathetic care within the community setting. A Band 5 Community Staff Nurse provides holistic, patient-centred care in homes. Duties include: comprehensive assessment and individualised care planning; diverse clinical interventions like wound care, medication administration, catheter care, and palliative support; autonomous caseload management, prioritising visits and documenting accurately. They communicate effectively with patients, families, and multidisciplinary teams (GPs, OTs, Social Workers) for integrated care. The role demands strong problem-solving, health promotion, safeguarding awareness, and continuous professional development, ensuring safe, effective, and compassionate nursing care in the community.
Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
