Exciting opportunities have arisen for  new Community Rehabilitation Assistants to join Poole Intermediate Care Service.

The chosen candidates will work as part of a Multi-disciplinary Team caring for patients with an illness in their own homes to prevent hospital admissions and also to facilitate complex discharges from hospital. You will work alongside qualified Occupational Therapists, Physiotherapists, Nurses, Therapy Technicians and wider locality teams. Good communication skills and a caring attitude are needed.

This is a full time post of 37.5 hours per week. Shift working is required as the service operates 365 days a year 08.00 to 20.00. The successful candidates will need to work some weekends.  Part time hours will be considered.

There is a need to use your car to visit patients in their own homes.  Car owner/driver essential subject to the provisions of the Equality Act 2010. Basic IT skills and NVQ level 2/3 would be advantageous

Main duties of the job

To contribute to the aims of the service by working to prevent avoidable hospital admissions and to support early supported discharges from acute and community hospital units.

To contribute to the objectives of the service by working to increase independence, reduce dependence on long-term health or social care and, where achievable, to avoid admissions to long-term care.

To work as part of an integrated, multi-disciplinary team delivering outcome-based packages of intermediate care to users of the service.

To contribute to the planning, organisation and delivery of packages of integrated, intermediate care, as agreed by a qualified practitioner.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
