Main duties of the job

  • To provide high quality evidence based nursing care in a community setting.
  • To actively participate in the planning and discharge of complex patients from secondary to primary care
  • work as part of multi-disciplinary team and in partnership with primary care and secondary care.
  • Can provide motivational and negotiation skills in areas where consent/ co-operation is required or there may be barriers to understanding. To include undertaking best interest decisions
  • To ensure timely and effective communication with the team manager and other team members on professional matters
  • To liaise with patients/relatives who could be unpredictable/or challenging in their responses because of stress related problem.
  • Strong communication is essential, as we work closely with a wider multi-disciplinary team, particularly our Intermediate Care Team, to support seamless, integrated care.
  • You must have an ability to provide and receive complex, sensitive and contentious information.
  • Empathy and reassurance skills
  • Ability to provide motivational and negotiation skills in areas where consent/ co-operation is required or there may be barriers to understanding.

Detailed job description and main responsibilities

To monitor and evaluate care programmes for patients and carers, promoting effective co-ordination and continuity of patient care.

To communicate formally and informally with the DN team lead to ensure best practice and consistency with the community and intermediate care service in TSDFT

An ability to provide and receive complex, sensitive and contentious information: using empathy and reassurance skills

To liaise with patients/relatives who could be unpredictable/or challenging in their responses because of stress related problems

To undertake Specialist Nursing and Continuing Care assessments (with appropriate training)
