Hywel Dda University Health Board are currently looking to recruit a full time Community Registered Mental Health Nurse to join the North Pembrokeshire Community Mental Health Team based at Bro Cerwyn, Haverfordwest.

The post is permanent and will be for 28 hours per week.

Main duties of the job

Our services work through an assessment & recovery approach with people accessing secondary mental health services for severe and enduring mental illness. With an exciting opportunity to be involved in an adapting and transforming structure to our mental health services.

The post holder will be responsible for the assessment of care needs, the development of programs of care and the implementation and evaluation of these programs of care for a defined clinical caseload including the role of a Care Coordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.

The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies, maintaining good communication across all boundaries.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
