Job overview

An exciting opportunity has arisen to join the Community Enablement and Recovery Team (CERT) across the Black Country. As a Community Mental Health Recovery Practitioner, you will play a pivotal role in providing community rehabilitation in collaboration with Community Mental Health Teams (CMHTs). You will work with a capped caseload of service users, who are either in out-of-area rehabilitation placements and due for discharge or residing within the Black Country with a diagnosed Severe Mental Illness and requiring community rehabilitation support.

Main duties of the job

The post holder will work as part of the Community Enablement and Recovery Team (CERT) MDT, which includes a Consultant Psychiatrist, Psychologist, Nurses, Occupational Therapists and Support workers.

  • Care Coordination: Manage and coordinate patient centred care for service users, ensuring their rehabilitation needs are met.
  • Provide wrap around care: Work in partnership with CMHTs, Local Authority, care providers and family
  • Hospital Discharge Support: Facilitate smooth transitions from hospital to community settings.
  • Recovery-Focused Interventions: Deliver tailored interventions aimed at building resilience, improving wellbeing, and addressing complex mental health needs.
  • Assessment and Planning: Assess individual needs and risks to ensure that care packages are appropriate and effective.
  • Intensive Support: Work closely with clients for up to 2 years, focusing on their individual, cultural, and social needs to help them achieve their optimum level of functioning and a fulfilling life.
  • Step-Down Services: Ensure a seamless transition to locality-based services when appropriate. The post holder will liaise with external agencies to achieve personalised recovery outcomes for patients within the service.

Some travel will be required to visit out of area placements to assess service users and to review care provision

The service will operate Monday to Friday 9-5 hours but is expected to advise service users and carers of access to services outside of these hours and liaise and refer on to other mental health care providers in cases where support outside of these hours is required.

Contribute to the development and implementation of the Community Enablement and Recovery Team

Working within an MDT to work holistically and improve our patient’s recovery journey

Liaison with external services to facilitate discharge from out of area placements, co-ordinate 117 packages of care and to support referral process
