This post will sit within our multidisciplinary community dementia team and will work as a lead practitioner to deliver interventions to those people who have more complex or intensive needs. You will also be required to work in Intensive support which is also part of the DOPMHS team so flexibility is key.  We aim to help people to remain safe and well at home and able to maintain the relationships that are important to them.

The service has an intensive support element within it that offers extended weekend interventions to people who are seen by the team.

The service operates 0900 - 1700 hours Monday to Friday. The Intensive Support also work weekends and bank holidays.
