Are you looking to work in an intervention led service to promote wellbeing among older adults with  mental health diagnoses? We are looking for Mental Health Nurses to be aligned to our older adult pathways of care which deliver memory assessments and intervention-based care for people with serious mental illness and/or dementia with associated behavioural or psychological needs.

West Kent MHT+ is a diverse multi-cultural team that is looking to recruit a Band 6 Mental Health Nurse with experience in working with older adults.  The successful applicant is to work within the team providing specialist mental health care for people with multiple and complex needs by offering interventions that promote wellbeing and recovery within an adult all-age service.

If you are interested in developing your skills and delivering clinical care for people with dementia and/or complex emotional difficulties, depression, psychosis and other mental health presentations we are the team for you.

Full training and support will be provided to ensure that you have the skills and confidence to deliver.

Main duties of the job

  • To act as a Practice Assessor.
  • To adhere to the NMC registration and revalidation requirements.
  • To adhere to KMPTs policies, procedures and guidelines.
  • To support and behave in a way that upholds KMPT Trust values.
  • To assess, act on and report any safeguarding concerns.
  • To optimise health and wellbeing; promoting self -care where possible, helping people to make choices about their needs and involve their carers where appropriate.
  • You may be required to manage, administer medications and monitor their effects.
  • You may be required to carry out specific duties relevant to the setting of community care  depot and clozapine clinics.
  • To provide interventions to promote recovery.
  • To assess risk and identify client’s needs.
  • To assess and monitor complex physical health needs.
  • To undertake or assist with cognitive and memory assessments.
  • To join work with other agencies involved in the client’s care, i.e. care homes, dementia support services, etc.
  • Recording and reporting accurately any changes related to the patient’s current behaviour and risks using red board meetings daily or to team managers and/or medics.
  • Recording care given in a timely manner as per professional and local standards using approved KMPT systems.
  • To supervise a range of other staff and their activities as delegated.
  • Acting as a role model and supporting new team members through demonstration and explanation of tasks and techniques.
  • Undertake and maintain all essential training to role

MHT+ (previously CMHT)  is an adult all-age service providing assessment, care and treatment to those with enduring mental illness.  This vacancy will be to support the older age clients within the service, however there may be times when you may be expected to support younger adults to meet the needs of the service.
