Job summary

Main duties of the job

ISTC are seeking an individual who is committed to delivering a high-quality service.

The successful applicant will demonstrate strong clinical and leadership qualities, working as part of an MDT approach, supporting service users across Calderdale.

Community Mental Health Practitioners within ISTC are expected to:

Provide home based treatment as an alternative to hospital admission where clinically viable

Reduce the length of inpatient stay

Improve patient choice and experience

Work for short, intensive periods with people who have acute or severe mental health problems

Contribute to the delivery of planned care in the service users own home, accessing community resources

You will have a passion for working within the field of older peoples mental health, as well as experience of working within a multi-disciplinary mental health setting.

The teamwork 8am 8pm 7 days per week and you would be expected to undertake 12-hour shifts across a 7-day rota.

Full UK driving licence is essential.
