Are you a physiotherapist wanting to enhance your community skills as well as your organisational and management skills?

An exciting opportunity has arisen to work within the community therapy team in St Albans and Harpenden. We are looking to recruit an experienced physiotherapist to the team. The role includes managing your own caseload as well as triaging new referrals, and the caseload includes- prevention of admission, supported discharge and core community therapy.

Our integrated community team comprises of therapists, nurses, healthcare assistants, and technical instructors as well as Voluntary sector advisors and team administrators

Successful applicants will be part of an enthusiastic, patient focused, team caring for patients in the community while working collaboratively with GPs, acute hospitals, Social care, Hospices and other third sector organisations.

Main duties of the job

  • Be an active member of the clinical team, providing a comprehensive therapy service to residents of West Hertfordshire referred into the service.
  • Work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation.
  • Build and maintain key relationships with referrers to the service, to facilitate appropriate referrals and an understanding of the role of the Physiotherapist and the Multi-professional team
  • To deliver appropriate treatment in a variety of settings including the patient’s own home, residential and nursing homes and day centres.
  • Be responsible for the supervision of junior staff, rehabilitation assistants and Physiotherapy students
  • Actively participate in Continuing Professional Development to support your own development and the development of the service. Including participating in any audit or research being completed by the service
