Job summary

Are you an enthusiastic Physiotherapist looking to develop your skills within a supportive community therapy team? We have an exciting opportunity for a Band 5 Physiotherapist to join our service, delivering high-quality, person-centred care to patients in their own homes and community settings.

As a Band 5 Physiotherapist, you will work as part of a multidisciplinary team to provide comprehensive rehabilitation and therapy interventions, supporting individuals to achieve their goals and maintain independence. This includes working alongside other therapy colleagues to deliver co-ordinated, high-quality rehabilitation. You will manage a varied caseload, including complex cases, and contribute to service development and quality improvement initiatives.

Applicants must be a HCPC registered physiotherapist with and have full access to a vehicle for work and possession of a full driving license is essential.

Main duties of the job

Work as part of the multi-professional Locality Health Team delivering programmes of rehabilitation within the community in close collaboration with other relevant services/agencies e.g. other community and inpatient teams, social care, primary and secondary care.

Manage a caseload with supervision as required/be responsible for a defined area of service, undertaking comprehensive assessment, planning and implementation of comprehensive programmes of care for patients who require intensive support / rehabilitation guided by more senior clinicians where required.

Participate in and maintain own clinical professional development (CPD) by keeping abreast of new developments and incorporating into practice under guidance from supervisor and providing supervision and assessment to rehabilitation assistants and students on placement.

Work collaboratively with partners in health and social care, to establish a whole systems approach to managing patients with complex care needs, maintaining a "community pull" on patients being admitted to secondary care
