The Discharge to Assess Team/Intermediate Care at Home Team are looking for a Registered Community Nurse, to fulfil an exciting opportunity within Stockport Foundation Trust. Discharge To Assess is a multi-professional service supporting patients to leave the hospital setting back into the community. The team provides individualised patient centred care to assess a person’s needs, whilst optimising independence and facilitate a safe transfer to onward care into the appropriate services: Health, Adult Social care or partner agencies.

The Intermediate Care at Home Team provide nursing and therapy rehabilitation to our Stockpot residents.

Main duties of the job

To support people who are clinically optimised and do not require an acute hospital bed, but may still require care services with short term, funded support to be discharged to their own home or another community setting.

Assessment for longer-term care and support needs is undertaken in the most appropriate setting and at the right time for the person.  We provide holistic multidisciplinary interventions to stabilise patients in their own home or usual residence. This comprises a multi-disciplinary integrated team.

Short-term rehabilitation support to optimise independence in the community with the aim to reduce unnecessary hospital admissions, enable early discharge from hospital and prevent unnecessary admission to long-term residential care. This role will require the post holder to be flexible to cover all areas within the Discharge to Assess service, covering 7 days 08.00-21.00.

Detailed job description and main responsibilities

The post holder will be a qualified Registered Nurse. You will be working within a multi-professional team incorporating Physiotherapists, Occupational Therapists, Nurses, Social Workers and rehabilitation Support Workers, as well as liaising with other community health and social care teams.

The whole service provides a person centred approach, which can be provided in a client’s own home. The service is provided across 7 days a week covering core hours of 8 am – 9 pm.

You will be based at Regent House, Heaton Lane, Stockport, SK4 1BS. The successful candidate will be expected to work independently, but also as an integral member of the multidisciplinary team. You will be offered 1:1s  and  access to supervision peer support. The service promotes regular profession specific and MDT team meetings.

You will need to be a Registered Nurse with an enthusiasm for working within the community, with the ability to work flexibly between all aspects of the service. We place a great emphasis on training and personal development and will provide you with access to training and supervision. The candidate will be able to demonstrate sound clinical reasoning, excellent time management and advanced communication skills. There will be support available from clinical leads. Lone working is an expected element within this community role with support from the Multidisciplinary team.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
