Job summary

Main duties of the job

You will need to be a car driver and have the use of a vehicle to undertake any necessary travel in connection with the duties.

A key element of the role will be care coordination, supporting colleagues, duty worker and prioritisation of workload in times of high clinical demand.

Undertake assessments and review care plans of service users which promote engagement, stability, recovery and risk management.

Manage a varied caseload covering functional and organic mental health presentations.

Work autonomously but contribute to the functioning of a multi-disciplinary team.

Work with service users with both short-term interventions and severe and enduring mental health needs to promote health and well-being.

Document care delivery to a high standard in line with professional standards, local and national guidance.

Recognises situations that may be detrimental to the health and well-being of the service user and initiates action that may be required.

Support student education when allocated to the team and to contribute to the preceptorship and education of newly qualified staff where appropriate.

Administration of depot medication in the community.

Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

We are a specialist NHS Foundation Trust that provides community, mental health and learning disability services for the people of Barnsley, Calderdale, Kirklees and Wakefield. We also provide low and medium secure services and are the lead for the west Yorkshire secure provider collaborative.
