Main duties of the job

Our Community Mental Health Teams (CMHTs) are dedicated to providing planned and effective mental health support and interventions tailored to the individual needs and goals of each service user. Care is delivered in partnership with service users, their loved ones, and other services to ensure a holistic and person-centred approach.

Collaborative Care

  • CMHTs work closely with other agencies and organisations to promote the health and wellbeing of the local population.
  • Responsibility for care is shared between the CMHT, partner organisations, the service user, and their loved ones.

Multi-Disciplinary Team (MDT) Approach

Our teams operate in a collaborative, recovery-focused way, guided by trauma-informed care principles. The MDT includes:

  • Consultant Psychiatrists
  • Clinical Psychologists
  • Registered Mental Health Practitioners (Mental Health Nurses, Occupational Therapists)
  • Community Support Workers
  • Peer Support Workers
  • Representatives from social care, housing, drug and alcohol services, and the voluntary, community and social enterprise (VCSE) sector

Keyworker Model
