Community Mental Health Nurse / Practitioner

We are looking for someone who has:

We offer a supportive induction and a welcoming team environment, and look forward to receiving your application!

Main duties of the job

Reporting into the Clinical Nurse Specialist, your responsibilities will include:

  • Manage your own caseload, conducting initial and comprehensive assessments, especially for those presenting high risk or experiencing rapid mental health deterioration.
  • Daily referrals meetings, where all CAMHS referrals are triaged as part of an multidisciplinary team.
  • Consultation calls to support young people awaiting initial assessments and professionals needing referrals.
  • Rapid response appointments for high-risk young people, offering follow-up support
  • Help develop care plans in collaboration with young people, including the views of families/carers, where appropriate.
  • Supervise and support junior colleagues
