Within the Bournemouth East Community Mental Health Team, an opportunity has arisen for a Band 6 Community Mental Health Practitioner to join the service on a full-time basis (37.5 hours per week, Monday to Friday, 9:00am–5:00pm), although part-time hours can be considered for the right candidate.
For the right candidate we could consider a developmental role from Band 5 to Band 6.
The successful candidate will work as part of a well-established, integrated multidisciplinary team comprising Psychiatrists, Nurses, Psychologists, Employment Specialists, Physical Health Support Workers, Social Workers, Administrative staff, and Support Time Recovery Workers.
This is primarily a care coordination role, with responsibility for supporting individuals with complex mental health needs throughout their recovery journey. The post holder will undertake comprehensive assessment, care planning, risk management, and multi-agency liaison to ensure safe, person-centred care delivery.

The team has strong values centred on recovery-focused and patient-led practice, ensuring that service users’ wellbeing remains at the heart of all interventions. You can expect regular supervision, peer support, and opportunities for ongoing professional development to promote safe and confident practice.
You will be a committed, dynamic, motivated professional with excellent communication, organisational skills and the ability to prioritise your work. You should have recent experience of working in a mental health environment and be able to apply the philosophies of engagement and recovery in your work with service users.

You will contribute to effective multi-disciplinary working within the community mental health service and to build up effective liaison with community, primary, and other secondary and tertiary services/agencies within the locality to provide holistic care based on the individual’s need and service development.
The team is based on the grounds of King's Park Community Hospital in Boscombe area of Bournemouth; the post holder will need to have the ability to travel by car, with business insurance, as they will be required to assess and monitor patients in their own homes and within other community or inpatient settings.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
