An opportunity has arisen for experienced registered nurses, in Hammersmith and Fulham Older People’s Community Mental Health Services, within local Cognitive Impairment and Dementia Services.

We are looking for dedicated, enthusiastic community nurses who is a highly motivated and caring professional, who can provide the best possible care with compassion to our patients. To thrive in this role, you need to be resilient and committed to adapt, working in a changing environment.

The successful candidate will have experience within Dementia Care, knowledge and experience of older adults with mental health needs or be able to demonstrate your transferrable skills within your application and will hold a valid, recognised mentorship qualification.

Main duties of the job

You will have post registration experience, in the specified care group, and will be someone who can make a significant contribution in supporting the delivery of high quality assessments, investigation and treatment, supporting the team in the day-to-day running of the service, whilst being expected to take a lead on a core aspect of service delivery.

The post holder, will work with individuals who present with a range of memory related health needs, by providing assessment, information, treatment, review and support through individual and group work as part of a multi-disciplinary team.

You will work as a Care coordinator for a group of patients, and be responsible for liaising with other professionals and organisations to ensure that patients’ physical, social and psychological needs are met in an integrated and holistic way.
