An exciting opportunity has arisen within the Tonna Community Mental Health Team for a Registered Mental Health Nurse (Band 6) to join our busy team. We currently have one full time permanent post available. The team operates Monday to Friday, 9am to 5pm except Bank Holidays.
We are looking for a motivated individual who can work independently and as part of the Multi-Disciplinary team. Excellent communication skills are essential as well as having a positive attitude to mental health care whilst ensuring individual needs for privacy, dignity, respect and confidentiality are upheld at all times.

The role will consist of working as part of a multidisciplinary team in undertaking the role of the Care Coordinator in the delivery of a high quality community mental health service under the framework of the Care Programme Approach and the Mental Health Measure (2010) Wales whilst maintaining the Recovery ethos at the forefront of nursing care.
To assess implement and evaluate agreed care and treatment plans for each individual on the caseload which has previously been agreed with the service users / carers. To work autonomously but under the indirect supervision of a Band 7 registered nurse / team manager and in conjunction with unregistered Band 3 healthcare assistants, other health colleagues and colleagues from the Local Authority in delivery of a supportive service for those people under the care of the CMHT.
There will also be an expectation of the successful candidate to work as part of our duty team, which will involve mental health assessments and providing a point of contact for existing service users in the absence of their care coordinator.
The CMHT also provides a variety of clinics, including depot, Clozapine, Lithium and physical health screening clinics, which the role will also be involved with in terms of providing support for substantive clinic nurses during times of leave.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
