Felixstowe Integrated Neighbourhood Team (INT) has a unique opening for a B7 Community Matron working 30 hours per week alongside other Health and Social Care professionals.
You will specialise in managing patients with long term conditions such as COPD and responding to acute medical episodes / exacerbation of these conditions.
You will also have completed specialist post graduate training with skills developed in advanced medical assessment with a non-medical prescribing element. You will need to work mainly autonomously and to work as part of the wider neighbourhood team.
There will be a need to build close working relationships with the three local GP surgeries as well as supporting the Felixstowe INT which consists of district & community nurses, generic workers, associate nurses, therapy assistant practitioners, occupational therapists, physiotherapists and Social Care Workers. The team is governed and supported by a Lead Nurse, Lead Therapist, Lead Social Worker and overall, by the INT manager.
The team is busy, dynamic and proud of their holistic care approach. We continue to change to support developments in NHS which includes close working relationships with the local statutory, non-statutory and voluntary services including GP, acute hospital colleagues, GP Federation, Social Prescribers, SuffolkMind, NSFT and many local community voluntary groups.
You will be actively supported by peers and medical supervision and will attend medicines management forums as well as an individual annual appraisal. You will be expected to work autonomously and on behalf of the patients served by the local area. You will have access to skills development training relevant to the post.
The role involves providing matron interventions to the housebound residents of Felixstowe and surrounding area, who are aged 18+ and registered with a Felixstowe GP.
Conditions include long-term neurological conditions, COPD, palliative and end of life care; diabetes; dementia, acute medical episodes,– all with a view to preventing admission and supporting early discharge. Many of our patients have a complexity of needs across physical, mental, psychological and social spectrums.
You need to have access to a car for work which is community based
Work autonomously as an advanced practitioner within the community, managing a caseload of patients delivering individualised direct patient care.
Direct responsibility for assessment, examination, investigation and diagnosis of patients within their area of work.
Appropriately treat patients, resulting in the safe management and appropriate referral or discharge of patients
Receive referrals via a variety of sources, including direct patient referral.
Manage own caseload and clinical priorities according to agreed protocols and working practices.
Participate in multidisciplinary patient reviews and multidisciplinary team meetings
Undertake a variety of clinical skills and provide treatment/ advice as per scope of practice, using agreed protocols of clinical practice and professional guidelines.
Within scope of practice and clinical competence request and / or undertake diagnostic procedures and clinical investigations related to plans of care
